What Is Clear Protection Insurance?

DynaFlex® Clear Protection Insurance provides coverage on clear retention cases associated with EZ-Clear & Defend® Retainers. 

When Can Clear Protection Insurance Be Purchased?

DynaFlex® Clear Protection Insurance is offered on EZ-Clear Retainers and Defend® Retention Systems. Simply select the insurance radio button next to the system you want insurance for in the DynaFusion® System. It’s that easy!

What Does The Insurance Plan Cover?

The Insurance covers free replacements of clear retainers for 5 years from the date of purchase. It applies specifically to clear retainers associated with EZ-Clear and Defend® Retainers.  

How Do I File A Claim Under The Clear Protection Insurance I Purchased?

Claims must be made through the DynaFusion® System using the claim Rx form and must be submitted by the doctor on behalf of the patient. Clear and accurate information must be provided when filing a claim.

How Often Can I File A Claim?

Consecutive Insurance Claims cannot be filed within a 30 day period. After 30 days, another claim may be filed if necessary. 

What Happens After A Claim Is Approved?

DynaFusion automatically verifies and approves the claim upon submission. Upon approval of a valid claim, DynaFlex will provide a free replacement clear retainer according to the specifications of the original Case. The upper and lower arches will be determined by what was purchased with the original order. The doctor can also decide whether to use the original scan or supply a new one for the replacement upon claim submission.

What Are The Limitations Of The Insurance Plan?

The Insurance Plan covers only clear retainers that are part of the original case and does not extend to additional or customized dental appliances. It also does not cover damage resulting from improper care, misuse, or neglect of the clear retainers. 

Will Digital Models Be Sent Back With The Retainer(s)?

No, digital models are not returned with insurance claimed retention devices.  

Will Shipping Fees Occur On Claims?

Yes, standard shipping charges apply to all insurance claims. Replacement retainers will be shipped to the subscribing office location.

Are Pontics Or Rests Included?

No, pontics and rests are not included in the purchase price of the insurance. However, for an additional charge, these can be added. Use the additional comments field on the claim form to request these items. 

Is The Insurance Plan Transferable?

No, the Insurance Plan is non-transferable and is only valid for the original patient and doctor who purchased the Case and Insurance Plan. 

Terms & Conditions

1. Introduction

The Clear Protection Insurance Plan (hereinafter referred to as “Insurance Plan“) is offered by DynaFlex to doctors for the benefit of their patients who have purchased a Clear Retention Case (hereinafter referred to as “Case”). By purchasing this Insurance Plan, doctors and their patients agree to the terms and conditions outlined below. 

2. Insurance Plan Coverage

2.1 The Insurance Plan provides coverage for a period of 5 years from the date of purchase. 

2.2 The insurance Plan must be purchased in conjunction EZ-Clear and Defend Retainers via DynaFusion Digital Rx System. Insurance is not available in conjunction with SmileShare Aligner Orders.

2.3 The Insurance Plan covers free replacements of clear retainers in the event of damage, loss, or wear and tear, as determined solely by DynaFlex. 

2.4 Standard shipping rates apply. 

2.5 Digital models will not be returned. 

3. Claim Process

3.1 Insurance Plan claims must be submitted by the doctor on behalf of the patient. 

3.2 The doctor must provide clear and accurate information when filing a claim.  

3.3 Claims may be denied if case number or patient information provided does not match the original order. 

3.4 Claims must be filed by submitting a claim Rx form through DynaFusion. 

3.5 If models are being mailed, please indicated in the additional instructions on the claim form (Select “Will Send New Scans” Option). Please note this may delay shipping times. 

4. Replacement Retainers

4.1 Upon approval of a valid claim, DynaFlex will supply a free replacement clear retainer for the same arch or arches (upper and/or lower) as the original order. 

4.2 Replacement retainers will be provided in accordance with the original specifications of the Case. 

4.3 The doctor must decide if DynaFlex is to use the original scan or if a new scan will be supplied. This is indicated on the claim form. 

5. Limitations

5.1 The Insurance Plan covers only clear retainers that are part of the original case. 

5.2 The Insurance Plan does not cover any additional or customized dental appliances beyond the clear retainers. 

5.3 The Insurance Plan does not cover damage resulting from improper care, misuse, or neglect of the clear retainers. 

5.4 Consecutive Insurance Plan claims cannot be filed within a 30-day period 

5.5 One additional retainer per arch as dedicated by the original work order will be shipped per claim submitted. 

5.6 Resets and pontics are not included, however these can be added for an additional charge. Please use work order comments on the claim form to submit requests. 

5.7 The Insurance Plan is not available in conjunction with SmileShare cases. 

5.8 The Insurance Plan cannot be discounted or combined with any other offers, promotions, or volume pricing. 

6. Transferability

6.1 This Insurance Plan is non-transferable and is valid only for the original patient and doctor who purchased the Case and Insurance Plan. 

7. Cancellation

7.1 The Insurance Plan cannot be canceled by the doctor once purchased. No refunds will be issued. 

7.2 DynaFlex reserves the right to cancel or terminate the Insurance Plan at any time in its sole discretion. 

8. DynaFlex Discretion

8.1 DynaFlex reserves the right to make the final determination on the validity of an Insurance Plan claim. 

8.2 DynaFlex may require photographic evidence, dental records, or other documentation to validate a claim. 

9. Governing Law

9.1 This Insurance Plan is governed by the laws of St. Charles County, Missouri. 

9.2 Any disputes arising from this Insurance Plan will be subject to the exclusive jurisdiction of the courts of St. Charles County, Missouri. 

10. Modifications

10.1 DynaFlex reserves the right to modify these terms and conditions at any time, and any such changes will be communicated to doctors who have purchased the Insurance Plan.
